The document must be dated within the last 3 months, and it must show both the person’s name and address. Acceptable forms of Address: HM Revenue and Customs Notice of Coding (preferred), Landline Telephone Bill, Utility Bill, Mortgage statement, Council tax bill, or Bank statement.
